Checklist

1. Three (3) copies of the application duly filled up.
2. Three (3) copies of the applicant's latest photograph, 2" X 2"
3. Certificate of employment (present employer)
4. Photo copy of current license or certification from PRC. (for safety engineer)
5. Photo copy of certificate(s) of occupational safety and health training/Seminars sponsored by the Bureau and/or recognized institution.
6. Processing fee of P 1,020.00 for temporary Safety Engineer/Inspector; P 1,520.00 for the permanent Safety Engineer / Inspector including Renewals as per Department Administrative Order 2000-98.
