Ore Transport Permit (OTP) Application

Applicant: Permittee or Trader

1. Fill-up application form for OTP (5 sets), and should be duly notarized.
2. Submit/present to the MGB Office the following:
   1. Permit number
   2. Buying Agreement/ Purchase Order
   3. Payment of excise tax, etc. (paid for the applied commodity tonnage)
   4. Local OTP for the applied tonnage
3. Submitted application will be evaluated by HEA or MMD
4. Payment of the following charges:
   1. Application Fee:

Non-Metallic Minerals ----- Php 1,000.00
Metallic Mineral ----- 2,000.00
PD 1856 ----- 20.00

b. Verification Fee ----- Php 6,000.00
c. Transportation Expense for verification team

5. Verification team makes documentation/report submitted to the Regional Director

6. Issuance of OTP form from RD's Office

7. Approval of OTP

8. Release of OTP (valid for 15 days from date of issue)
